      • Appointments are booked in 30 minute increments. All you need to do is arrive at least 5 minutes prior to your appointment time.
      • Once you schedule an appointment, you will receive a confirmation number via e-mail. You can cancel your appointment using your confirmation number and the same link.

Walk-Ins / Stand-bys

Mondays – Fridays, 8 am – 12 pm

        • Personnel from Los Angeles AFB will be able to come in at any time during duty hours for CAC emergencies only (i.e. CAC locked, lost or damaged CAC).
        • All walk-in customers are assisted on a first come, first serve basis. Wait times for walk-ins can be up to 2-3 hours. To avoid the possible wait, schedule an appointment.

Online ID Renewal

Step 1. Fill out attached “Application to Renew” form

Step 2. Sponsor Certify DD Form 1172 On-Line
Have the sponsor (CAC holder or Retirees can create a DS log-on) access the DMDC website to certify the DD Form 1172 for the dependent who is renewing his/her ID card via the following link:

Step 3. Submit Pictures of Dependent and 2x Supporting IDs via AMRDEC SAFE
Open AMRDEC SAFE and submit the following documents to the 61 FSS/FSMPS at

  1. Two supporting identifications (2 forms of unexpired ID with the EXACT same name, front only)
  2. Photo of Dependent/Retiree for the ID card (shoulders up, white/off white background)
  3. Completed “Application to Renew” form

NOTE: Reference the attached Supporting ID Requirements for additional guidance on supporting identifications and pictures.

4. Pick up ID Card at MPS Customer Service Office
The ID Card will be ready for pick up in 7 days. Must bring the original two supporting identifications used to create the ID card with you upon pick-up. Customer will sign ID card and DD Form 1172 acknowledging receipt. The sponsor is not required to be present for ID card pick-up.
